How to Request Your Health Records

1. Submit a written request by emailing Mixa@MeaningfulTalkTherapy.com with the subject line “Health Records Request” and include:

- Full Legal Name

- Date of Birth

- Date range of records requested

- Description of records you are requesting (if specific)

- Preferred delivery method (Email or Mail)

- Your Phone number

2. I may contact you to verify your identity prior to releasing records.

3. Requests are processed within 15 business days in accordance with Texas law.

Please note: Reasonable fees may apply as permitted by law, and you will be informed of any fees before records are provided.
